Application Process

Notice: Applications for the 2013 - 2014 Michigan Conrad 30 J-1 Waiver Program are available as of June 10, 2013 upon request as outlined below. 

An International Medical Graduate (IMG) who has an offer of full-time employment at a healthcare facility in a designated healthcare professional shortage area or at a healthcare facility which services patients from such a designated area may apply for a waiver through the Michigan Conrad 30 Program.

 

STEP 1: Request Michigan Conrad 30 application from the Michigan Health Council (MHC).

  • Make your request in writing including the physician’s full-name, professional acronym (MD/DO), address, email, phone number, specialty and graduation date from training.
  • Mail your request (email requests are not valid) and a non-refundable application fee of $100 to:

  • You will receive a personalized cover letter and application packet via email.

 

STEP 2: Apply for a U.S. Department of State Case File Number.  Click here for Department of State application instructions.

 

 

STEP 3: Complete the Michigan Conrad 30 application packet.

STEP 4: Submit the completed application to MHC. 

  • Mail your completed Michigan Conrad 30 application packet along with two (2) copies and a check for $1400 (to cover a non-refundable $200 application fee and $1200 processing fee) to:

  • MHC processes completed applications and forwards on to the Michigan Department of Community Health.

 

STEP 5: The Michigan Department of Community Health (MDCH) reviews applications.

  • MDCH sponsors 30 applications based on pre-established priority criteria.  Click here to view priority criteria.  PDF
  • MDCH sends out sponsorship or denial letters to all physicians/attorneys that apply.

 

STEP 6: The U.S. Department of State (USDOS) reviews sponsored applications.

  • Notice sent to physicians/attorneys when applications are forwarded to United States Citizenship and Immigration Service (USCIS).

 

STEP 7: USCIS grants final approval.

  • Letter sent to physicians/attorneys granting or denying waiver.
